  • Minor injuries have reduced but serious injuries and fatalities (SIF) have stayed the same or increased
  • Traditional measures are misleading and incomplete

Exploring solutions

  • De-emphasise the risk matrix – real risk is determined by exposure to uncontrolled energy sources and the presence and effectiveness of controls
  • Re-define controls – direct controls and enablers
  • Focus on how the controls work in the real world – acts, objects and engineered systems
  • Re-position health and safety as the presence of effective controls
  • Re-imagine the hierarchy of control
  • Re-think how to monitor and measure performance
